For all types of claims
- Duly completed ‘Claim Request Form’
- Original Life Insurance Certificate (and Rider endorsement letter for rider benefits)
- Other supportive documents (if required)
For Death benefit
- Copy of Death Certificate or original Autopsy report
For Total and Permanent Disability (TPD) benefit
- Confirmation Letter and Medical Reports/treatment records about Total Permanent Disability issued by registered hospital or competent authority.
For Critical Illness benefit/ Active Care benefit/ Health Care benefit (For reimbursement benefit) – as relevant
- Statement of attending doctor (physician/surgeon or relevant specialist)
- Recommendation letter of specialist doctor(s) who treated (or) Ward-in-charge professor from which Life Insured was admitted.
- Confirmation Letter/medical documents by hospital, clinic, laboratory, or a competent authority, as the case may be (if relevant)
- Original medical bills (for reimbursement benefit)

You can go to the bank in person and pay according to the selected premium payment amount. By transferring money to Dai-ichi Life Bank account or you can pay by standing order.
Premium payment service in cash or by cheque is available at KBZ Bank, AYA Bank, CB Bank, MOB Bank and Yoma Bank branches.
- Bank Branches: Visit the nearest branches of KBZ Bank, AYA Bank, CB Bank, MOB Bank, or Yoma Bank to make your premium payment in cash or by cheque. Inform the bank teller that you would like to deposit the premium to Dai-ichi Life Insurance Myanmar and provide them with either the Application ID or Policy ID. For cheque payment, it is required to do cheque clearing process as per banking process.
KBZPay, AYAPay or MAB Agents: Visit the nearest agent to deposit your premium to Dai-ichi Life Insurance Myanmar. Make sure to provide them with the Application ID or Policy ID.

These steps will guide you through the process of making premium payments using the respective banking applications.
To provide convenience and save you time when making premium payments, we highly recommend that our customers set up a standing order or direct debit arrangement through the following banking services:
- KBZ Internet/Mobile Banking,
- CB Bank Internet Banking or Branches
- MAB Internet Banking Service
- MOB Bank Branches
Deposit premium at the Customer Service counter or Dai-ichi Life Experience Centre (DLEC) in cheque or up to 200,000 MMK per transaction in cash for Htar–Wa-Ra Edu Goal.
Please note that our Agents do not collect cash from customers.
